Evolve Gynecology accepts a wide range of insurance plans and also offers transparent self-pay options to keep high-quality care accessible for women in Las Vegas and the surrounding communities. Patients can review accepted insurance providers, explore self-pay and out-of-network choices, and find clear guidance on billing to help plan their visit with confidence.
Accepted Insurances
Evolve Gynecology accepts a variety of insurance plans to help make care more accessible for our patients. If you have specific questions about your plan, please call the Member Services number on the back of your insurance card.
Self-pay options and Out of network benefits
We are unable to contract with all insurances. Many patients also have very high deductible plans and are often already paying out of pocket for much of their care even if they have an insurance. If you prefer to self-pay or use out of network benefits, please see our fee schedule below. Prices DO NOT include the cost of additional recommended labs, imaging, or additional visits, if recommended. Services must be paid in full prior to your visit or procedure.
Check your out of network benefits using Reimbursify, and please also call your insurance carrier prior to your appointment for more specific plan details. Our staff is unable to confirm your out of network benefits. We are happy to submit to your insurance on your behalf through the Reimbursify platform, and any reimbursement payments would be sent directly to you from your insurance company. All labs and imaging orders can be submitted through your insurance if preferred.

Our staff is available to answer questions you may have about insurance, billing, or payment plans. If you are an established patient with a billing question, please call our billing help desk at 725-777-4918. We accept cash, Care Credit, and most major credit cards. All fees for co-pays, co-insurance, deductibles, and balances accrued from prior visits are due prior to services being rendered. Failure to pay your balance will result in being sent to collections and subsequent dismissal from our practice.
